The following statement outlines The Earlsdon Practice’s (the Practice's) policy concerning the personal information it collects about you. It, along with our Disclaimer statement, forms part of our terms and conditions.

 

It is important to the Practice that our customers/patients understand what information we gather about them and how we use that information. The Practice abides by the principles laid down by the Data Protection Act 1998. Our aim is to be responsible, relevant and secure when using the information we collect about you.

 

Collected information and use

We collect contact details (name, postal address, post code, telephone/mobile numbers and email address) for all of our patients and anyone who requests information related to and/or about the services that the Practice offers. We do this so we can contact you about your treatment/appointment/s and/or to send you information about the Practice and its services, and things related to them. 

 

In the course of treating you we also collect general health and medical information about you (separate to the information collected via this website) so that our practitioners can treat you appropriately and in an informed manner (including making referrals). We will only use the personal information you provide us with for these specific purposes.

 

We may also collect information about how you interact with our website through the use of "cookies" placed on user machines. A cookie is a piece of software that a Web server can store on a PC to identify visitors should they visit the website again. While not all of the information that we collect from Users is personally identifiable, it may be associated with personally identifiable information that Users provide us through our website.

 

Disclosure of information to third parties

We will only ever disclose your personal information if we are required and compelled to do so by law, or if it will improve the service, treatment and information we provide to you.

 

We will never disclose your personal information for any other reasons than those stated above without first gaining your permission.
